Why are Credit/Debit Card Authorization Forms Important?

By obtaining a signed and completed Credit/Debit Card Authorization Form for a transaction, the business confirms with the cardholder that everyone is happy with the transaction. Therefore, if there's any dispute for the reasons listed below, the signed Credit/Debit Card Authorization Form would be provided as evidence when responding to that dispute.

Per Visa/Mastercard/Discover/Amex rules, a customer can dispute a transaction for the following reasons:

  • The transaction was never authorized by the cardholder.
  • The product was not received or service was not provided.
  • The quality of product or service did not meet the expectations of the cardholder.
  • The amount charged is different than the amount that was authorized.

Obtaining a signed and completed Credit/Debit Card Authorization Form provides documented evidence that the customer agreed to pay for the product or service, received the product or service, and is satisfied with the product or service. 

We highly recommend that full card data (card number, expiration date, cvv code) not be saved on the Card Authorization Form. Instead, the full card data should be provided at the time of payment (face-to-face or keyed-in), or the card can be recharged within PayJunction.
